Study Brokers – Where do they fit into the landscape?

As Director of Business Development for Rx Trials, I am responsible for acquiring new study opportunities for all of our sites and for ensuring that each site has enough appropriate trials to sustain their operation.  In the past several months, my curiosity has been peaked, about the concept of study brokers.  In speaking with many sites I have learned many sites have actually used study brokers in an attempt to increase their study opportunity, however, I’m also not hearing that the results have matched the promise. more...

We used a study broker on one occasion and it was well worth it. First it was a sponsor we had never done business with and the relationship has continued beyond that initial study (no broker fees had to be paid beyond the initial trial). Secondly, we only had to pay a percentage of what we earned so the risk was shared. The checks came directly from the sponsor to us, we negotiated the budget and contract. I would do it again, but with this same broker.

I have filled over a million (I am sure I not exaggerating :)) feasibility questionnaires for brokers over the past 11 years. To date we have received 2 good studies. Most recently though the study we received was from a CRO and a sponsor that we have worked with extensively and continue to get studies from both in the same disease area. 
 
My questions are this: 
 
Why if we are a very good site for the sponsor that we can't work directly with them, saving us all? 
 
If I fill out one questionnaire why can't the 90% of the questions that are the same stay in some database of the broker? 
 
 
 
We started looking to brokers this past year more to try to increase the different areas that we can get studies for and have yet to come up with anything very promising.  
 
Seems there should just be a more efficient way to tag sites with their capabilities and have them rated WITHOUT paying some high priced subscription that has been presented to me before. 
 
We certify study coordinators and investigators and CRA's; how about sites??
